Subject: Information Request for Respirator Audit Logic Database Reminder
At the manufacturers meeting held on October 11, 2006, NIOSH/NPPTL informed the respirator manufacturers that the original deadline for submitting responses from the NIOSH Approval holders was extended until December 2006.
A respirator list was sent in August 2006 to each NIOSH Approval holder which indicates all the NIOSH Approval Numbers currently on file for each company at the Technology Evaluation Branch. The Technology Evaluation Branch is requesting that all NIOSH Approval holders review their enclosed list of NIOSH Approval Numbers and respond to the following:
Product Status – (active, inactive and/or obsolete)
Primary Manufacturing Site Address for each NIOSH Approval
ISO Registered Facility (yes/no and identify registered to what standard e.g. 9001, etc.)
The information being requested is vital in developing an operational database for the Respirator Audit Logic Post Certification Audit Selection.
If you have already completed and forwarded the information request to NIOSH/NPPTL, you need not respond to this reminder. For those manufacturers which have not responded a deadline of no later than December 31, 2006 was established for receipt of this information.
