Methods for State Oral Health Surveys that began data collection in 2005

Alabama
The survey was conducted during the 2005-2006 and 2006-2007 school years. The sampling frame consisted of all public elementary schools in Alabama. A stratified probability-proportional-to-size sample of 135 schools was selected and 124 agreed to participate. Children were screened using passive consent. The statewide response rate was 73%. The survey followed the methods outlined in ASTDD’s Basic Screening Survey. In the state, 56% of elementary schoolchildren are eligible for the free/reduced lunch program (FRL); among students attending schools in the sample, 51% were eligible for FRL. The data were weighted for the sampling scheme and adjusted for nonresponse.
Alaska
The survey was completed during the 2005-2006 school year. The sampling frame consisted of on-site elementary schools with a kindergarten enrollment of 10 or more (correspondence and distance education schools were not included in the sampling frame). With implicit stratification by percent of students eligible for the free/reduced school lunch program, 14 schools were randomly selected and agreed to participate. Five dentists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the students selected, 60% responded. The data were self-weighting and the estimates presented are not adjusted for nonresponse.
Iowa
The survey was conducted during the 2005-2006 school year. The sampling frame consisted of public elementary schools with 3rd grade. A random sample of 29 schools was selected and all 29 agreed to participate. The screenings were completed by 25 dental hygienists using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the students selected, 68% responded. The percentage of participating children eligible for FRL was 31% while 37% of students in the state were eligible for FRL. The estimates were not adjusted for nonresponse.
Michigan
The survey was conducted during the 2005-2006 school year. The sampling frame consisted of public elementary schools with at least 20 students in 3rd grade. Probability proportional to size sampling was used to select 76 elementary schools, of which 74 agreed to participate. The screenings were completed using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Only children who returned a positive consent form were screened. Of the students invited to participate, 68% responded. The percentage of participating children eligible for FRL was 40% while 38% of elementary students in the state were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.
Montana
The survey was conducted during the 2005-2006 school year. The sampling frame consisted of all public elementary schools with at least one child in 3rd grade. A stratified probability sample of 30 schools was selected and all agreed to participate. Only children whose parents provided positive consent were screened. The response rate was 90%. The diagnostic criteria were comparable to ASTDD’s 1999 Basic Screening Survey. In the state 35% of students were eligible for the free/reduced lunch program (FRL) while 41% of the students in the participating schools were eligible for the FRL program. The estimates presented are weighted to account for the survey design.
Nevada
The survey was conducted during the 2005-2006 school year. The sampling frame consisted of all public elementary schools with a 3rd grade enrollment of 20 or more. The sampling frame was stratified by region with implicit stratification within each region by percent of students eligible for the free/reduced school lunch program (FRL). Forty-one schools were selected and 34 agreed to participate. Positive consent was used with a response rate of 22%. Trained Oral Health Program staff along with volunteer dental students and residents completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. In the state, 41% of public elementary school students participate in the free/reduced lunch program (FRL) while 44% of the students in the participating schools were eligible for FRL. The estimates presented are adjusted for the sampling scheme and nonresponse.
South Dakota
The survey was completed during the 2005-2006 school year. The sampling frame consisted of all public, private and Bureau of Indian Affairs elementary schools with a 3rd grade enrollment of 3 or more. With implicit stratification by percent of students eligible for the free/reduced school lunch program (FRL), 33 schools were randomly selected and 32 schools agreed to participate. Volunteer dentists and hygienists completed the screenings using diagnostic criteria comparable to ASTDD’s 1999 Basic Screening Survey. Of the students selected, 66% responded. The data were self-weighting and the estimates are presented adjusted for nonresponse.
