No, there is not a font size or style requirement. However, it is recommended that a font size of 11 points or greater is used.
Performance Narrative Requirements for the first half of the Current Budget Period (June 29, 2020 – December 31, 2020) are noted below:
- Recipients are required to submit an Annual Progress Report for the Year 1 Performance Narrative. The Annual Progress Report (which should not exceed 10 pages) must include all required elements for each Strategy. These elements are listed in the Continuation Guidance and include the work or key deliverables conducted during the first half of Year 1, successes, challenges, and technical assistance needs (if applicable).
- Recipients are not required to submit a revised Year 1 Work Plan to include with the Year 1 Performance Narrative.
It is recommended that Recipients discuss with their Project Officer any needed revisions to incorporate into the required Year 2 Work Plan(s) for the New Budget Period (April 29, 2021 – April 28, 2022).

These elements are listed in the Continuation Guidance and include the work or key deliverables conducted during the first half of Year 1, successes, challenges, and technical assistance needs (if applicable).

The Annual Progress Report, Work Plan(s), and Budget(s) each should be uploaded as separate documents and labeled using the naming conventions outlined in the Continuation Guidance. It is recommended that Recipients submit an Excel Document for the Work Plan.

The Recipient is required to submit the PPMR cover page. In addition, please fill out any portion of the PPRM Forms A – F that you can while taking into consideration that the deadline for many of the items are not due until April 2021. Upon submission, CDC will review and determine if other information is needed.

Recipients may use a paragraph or table format, as long as the Annual Progress Report does not exceed 10 pages total for both Component 1 and Component 2 combined. The Annual Progress Report must include all required elements for each Strategy. These elements are listed in the Continuation Guidance and include the work or key deliverables conducted during the first half of Year 1, successes, challenges, and technical assistance needs (if applicable).

Templates will not be provided for Component 1 or Component 2 Year 2 Budgets. Recipients must submit a separate budget justification for the Year 2 budgets based on the funding amount requested. The separate budget justification must be prepared for Component 1 and Component 2 with the level of detail as described in the CDC Budget Preparation Guidelines (located in GrantSolutions).
Templates will not be provided for the Annual Progress Report or the Year 2 Work Plan. Please review Page 2 of the Continuation Guidance to locate templates for the other required forms of the Application Packet.
Year 1 Performance Narrative: Annual Progress Report (June 29, 2020 – December 31, 2020)
For each Strategy, it is recommended that Recipients provide a brief summary for Component 1 and/or Component 2 (not to exceed 10 pages combined) of work or key deliverables conducted thus far, successes, challenges, and technical assistance needs.
Year 2 Work Plan (April 29, 2021 – April 28, 2022)
Recipients are required to submit Year 2 Work Plan(s) for Component 1 and/or Component 2 that reflect the new performance period. It is recommended that Recipients start with their existing Year 1 Work Plan and make edits and updates as appropriate to that document to then finalize and submit as their Year 2 Work Plan.

No, it is not required. However, you can submit an Interim FFR for review if you anticipate there will be unobligated funds for Year 1.

Unobligated funds may be used for purposes within the scope of the project as originally approved. Recipients will report use, or intended use, of unobligated funds in Section 12 “Remarks” of the annual Federal Financial Report. Provide a breakdown of estimated unobligated by component in the remarks section. If the Grants Management Officer (GMO) determines that some or all of the unobligated funds are not necessary to complete the project, the GMO may restrict the recipient’s authority to automatically carryover unobligated balances in the future, use the balance to reduce or offset CDC funding for a subsequent budget period, or use a combination of these actions.

AMP will not be used for the Continuation Process; therefore, no information related to the preparation and submission of Continuation Application materials will be posted in AMP. AMP remains available for technical assistance requests, downloading other tobacco prevention and control resources, and downloading guidance for the evaluation plan and the data management plan.
