CDC-RFA-DP20-2001: National and State Tobacco Control Program

For this Continuation Application, Recipients are only required to submit an Annual Progress Report for Component 1 and Component 2 that covers the time period of January 1, 2021 to December 31, 2021.  A separate six-month report is not required.

No, it is not required to populate the “Notes” box for every Activity in the Year 2 APR, but a “Status” is required.  If an Activity is incomplete, the Recipient should use the “Notes” box to provide additional context.

The Recipient should also use the Successes and Challenges Boxes to highlight 1) progress/successes for Activities that significantly contributed to the outcome for each Strategy and 2) challenges for Activities that will not be included in the Year 3 Work Plan.

No, Recipients should use the Year 2 APR Template to report all progress related to each Strategy for the time period of January 1, 2021 to December 31, 2021.  If a Year 1 Strategy is no longer available on the Year 2 APR Template, the Recipient can submit an additional document labeled “Other Year 1 Strategies” to highlight the successes achieved between Jan 1-April 28, 2021 for the deleted Strategy.

Administrative reports are Progress Reports or any reports that are required by the Recipient for Program evaluation.  It should be noted that CDC will allow flexibility with the page limit requirement for Work Plans and Annual Progress Reports exported from AMP.
