CDC-RFA-DP18-1816: Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN)
WISEWOMAN (Well-Integrated Screening and Evaluation for WOMen Across the Nation). This 5 year cooperative agreement funds 24 awardees in 21 state health departments and 3 tribal organizations. These programs serve low-income, uninsured, and underinsured women ages 40 to 64 by extending preventive health services including assessment of cardiovascular risk factors and provision of services to reduce those risks through improved diet, physical activity, tobacco cessation, and medication adherence support. For the first time, CDC has included an innovation component awarded to a subset of awardees that receive funding through the core award to support the implementation and evaluation of a small set of innovative strategies designed to reduce risks, complications, and barriers to the prevention and control of heart disease and stroke.
